Hi Kaitlin,
It sounds like you're dealing with a lot of stuff inside and outside of school that's creeping into your academic life which is completely normal. It's incredibly hard to maintain a high standard of work if you have heaps of other stuff going on in your head. But have you spoken to anyone about your issues? I'm sure your school has a counsellor - speak to them. You shouldn't have to deal with this all on your own. Also, could you speak to your teachers about what's going on? You don't have to go into specifics but just to let them know that you're going through a rough period at the moment. Maybe they can cut you a bit of slack.
I feel like you're beginning to see school as a threatening environment rather than just a place to learn. If that's the case then you need to take some steps to reduce that fear. Speak to somebody about this - don't hold it all in to yourself.

I am sorry to hear that you've been feeling anxious. When I was still in school, I struggled with severe anxiety too, so I know exactly where you're coming from.
Have you spoken to anyone about how you're feeling? Consider your parents, a trustworthy and supportive friend, a teacher or a school councillor. I cannot stress enough how important it is to have an outlet during tough times - whether it's talking to someone or getting it out on paper.
The first thing you should do is visit your GP for a professional diagnosis and a referral to a psychologist.
