Hello (Lisaftm)Lucas,
Growing up with siblings can be rough, mine were both quite unwell (physically) as children and spent a lot of time in different hospitals/specialists etc. So I can understand how your own issues can seem to appear swept under a rug or tucked to one side.
You are welcome to open up on here as much as you feel comfortable and the help lines Sophie_M has listed are great people to talk to. It sounds like your friend alice is an awesome person, there are more like them out there.
Depending on which state or territory you are in there may be some local council services you can get in contact with to meet with other transgender people as a peer group you can talk with/ have coffee etc as and idea to look a little further into about life etc. not just the dysphoria/surgery/stereotypical subjects.

Hi Lisaftmlucas,
Thank you for such a raw and open post, I'm so sorry to hear your having a troubling time.
It was hard enough for myself coming out as gay in 2003, I couldn't imagine the difficulties your going through you your current journey of life.
All I can say is to not give up and stay strong, you'll get through these hard times coming out a much stronger person!
We're here for you!
Regards,
D

Hello Dear Lisaftmluca,
A very warm and caring welcome to our forums..
Im sorry your struggling so much with you feeling unsettled and suicidal...You have been through a lot and still are..
Im wondering and would like to gently ask you if you have any mental health professionals caring for you...and if it’s something that you would consider....suicidal thoughts really need to be talked out with a professional to help you find out and understand what’s happening to you...
Sleep is vital to restore our body and mind..finding it hard to sleep with an overactive mind is very difficult...Do you listen to soft music, meditation or even sleep stories as your trying to sleep...Listening very mindfully to something calming might settle your mind into a sense of calm..which hopefully you can drift of to sleep while listening to them...
It takes a lot of courage to write out your first post..Well done Lucas
Talk here anytime you feel up to it...We are here for you..with the hope that we can support you through this hard time your going through..
My kindest and most caring thoughts..
Grandy..
