Centrelink Form for 3 monthly review

Does anyone know if the required form is able to be downloaded or do I need to obtain it from Centrlink?

Do you mean the medical certificate? If so, your gp will have it.

hi JL, google this 'how to get a Centrelink 3 monthly form' and then click onto Request a document using your Centrelink online account where you can download this for free from the Adobe website, see if this helps.
Mind you if you have registered with Centrelink for online services, good luck if you can get it to work, otherwise ring Centrelink and they will send out the form. Geoff.
